– يُعتبر انفصام الشخصية من الإضطرابات النفسية الهامة والتي تصيب 1% من البشر وتصيب الرجال والنساء في مقتبل العمر بين 18 – 25 سنه.

– يعتبر انفصام الشخصية إضطراب ذهاني تتأثر فيه قدرة المريض على الإتصال بالواقع بسبب الأعراض التي تؤثر عليه، ومنها الهلاوس مثل سماع أصوات غير موجودة تتحدث عنه أو تعطيه أوامر وتهدده، ورؤية أشخاص لا وجود لهم أو أشكال أو أضواء، وقد تترافق مع هلاوس شمية فتكون هناك روائح كريهة يشمها المريض ولا يشمها غيره، وتكون لأشياء معروفة مثل التراب والمسك أو غير معروفة مثل رائحة الموت أو الغدر، وهناك هلاوس تذوقية يكون فيها طعم غريب في الأكل والشراب، وهذا الطعم غالباً ما يكون للسموم والزرنيخ وأحياناً طعم السحر والحسد، وأكثر الهلاوس شيوعاً هي السمعية، ومن الهلاوس التي تحير المريض ومن حوله الهلاوس الحسية وهي أن يشعر المريض أن هناك من يلمس جسمه وأعضاءه، وأحياناً تصل للشعور بأن هناك من يجامع المريض، وقد يستخلص المريض أنه متزوج من جنيه أو أنها متزوجه من جني وقد يعلمهم بذلك أحد المشعوذين.

– العرض الهام الثاني لانفصام الشخصية هو التوهم، ويقصد به الإعتقاد الخاطئ الراسخ المسيطر والذي لا يقبل التغيير بأي وسائل منطقية، ولا يتمشى مع الخلفية الثقافية والعلمية والإجتماعية للمريض، وقد يكون على شكل التوهم بأن هناك من يضطهده ويراقبه ويتجسس عليه ويرهبه، إما أفراداً من المحيط أو جهات رسمية أو شعبيه أو أجهزه الأمن المحلية والعالمية، ويتبع ذلك التوهم بالإشارة للذات وفيه قناعة راسخة أن كل من يضحك أو يتهامس أو يتكلم مع غيره يقصده بالسوء، وقد يصل الأمر أن الإنترنت والصحف والإذاعة والتلفزيون تشير إليه ولو بصورة غير مباشرة وغالباً بما يؤكد هذا من أفكاره الإضطهادية ويتبع ذلك توهم العظمة وقد تكون غير مباشرة ناتجه عن أنه يتعرض لكل هذا الإضطهاد، فالعالم كله يضطهده إذن لأنه صاحب رسالة أو متميز أو عبقري أو أنه مرسل من رب العالمين، وقد يكون هناك توهم العدم وهو التأكيد على أن الجهاز الهضمي قد اختفى أو أن القلب قد توقف أو أنه ميت، وقد تكون أوسع من ذلك بأن العالم انتهي، وتوهم الذنب وفيه قناعه بأنه مذنب ذنب كبير وخطير وأثر على البشرية أو أنه سرق أو قتل دون أي رابط وعلاقه له بالجريمة.

– ومن الأعراض المزعجة لانفصام الشخصية أن المريض يعتقد بأن أفكاره تبث للعالم وأن كل الناس يعرفون أفكاره وكأن عقله محطة إذاعة، أو أنهم قادرين على قراءة أفكاره أو على زراعة أفكار في دماغه أو التحكم بأفكاره وسحبها وترك عقله في فراغ، وهذه أفكار مربكه وتثير الكثير من التغييرات السلوكية والإجتماعية

– يعتقد المصاب بانفصام الشخصية أن هناك قوة خارجية تسيطر عليه وتتحكم في تفكيره وانفعالاته وتصرفاته وأنه مسلوب الإرادة أمام هذه القوة، وقد يحدد هذه القوة بأنها شخص أو جهة معينة وقد لا يعرف من هو الذي يتحكم به.

– ومن الملاحظ أن معظم مرضى انفصام الشخصية يفتقدوا البصيرة ولا يقِّروا بمرضهم مما يشكل صعوبة في قبولهم للعلاج والمتابعة، وأنَّ اقتراح العلاج يضاف إلى الاضطهادات السابقة، وهناك مظاهر أخرى في تغيير السلوك والعزلة وإهمال النظافة وإهمال الواجبات اليومية، وقد يبدو على المريض اللامبالاة وتبلد المشاعر، وقد يشاهد من قبل الآخرين يبتسم أو يضحك بلا سبب أو يكلم نفسه وأحياناً يتمتم، وقد يختلط كلامه ويصبح غير مفهوم وقد يبدو وكأنه جمل متفرقة، يصعب الوصول للخلاصة منها.

– يتأثر تركيز المريض بانفصام الشخصية وقد يبدو مشتت الذاكرة مما يؤدي للفشل الدراسي، أو تدهور الأداء في العمل، وقد تظهر تصرفات يميل فيها المريض للعنف أحياناً خصوصاً أذا أهمل علاجه. في انفصام الشخصية المزمن قد لا تكون هذه المظاهر هي الأساس، ولكن ما يسمى بالأعراض السلبية، مثل اللامبالاه والتبلد وعدم المشاركة وعدم والحماس وحتى عدم الإهتمام بما يجري حوله، إذ يقضي المريض وقته لا يعمل شيء سوى التدخين ومشاهدة التلفزيون، وأحياناً المشي في الشارع لساعات طويلة يومياً ويتوقف أداءة في الحياة العملية والإجتماعية، وتجد الأم غير مكترثة لأطفالها ولا زوجها ولا بيتها مما قد يؤدي للطلاق، ويكثر هؤلاء المرضى من شرب القهوة والشاي بالإضافة للتدخين بشراهه، وقد يتعاطوا مواد أخرى إذا توفرت مثل المنشطات والحشيش مما يزيد المشكلة تعقيد أو يؤدي لإنتكاسات شديدة بين الحين والأخر، وهؤلاء المرضى تضعف قدراتهم ويبدو عليهم التراجع في فهم الأمور اليومية وحتى خبراتهم السابقة، وقد يصابوا أحياناً بتقلبات المزاج وهناك تلاقي عند أخرين بين انفصام الشخصية وبعض الوساوس القهرية أو الرهاب الإجتماعي والقلق.

– الأسباب التي تؤدي لانفصام الشخصية متعددة وفيها الإستعداد الوراثي ومشاكل الحياة وأحداثها وضغوطها وثم التغييرات الكيماوية في الدماغ نتيجة لذلك، وقد يبدأ المرض بشكل مفاجئ أو متدرج وبطيء على مدى شهور، وكثيراً ما يتأخر المريض في العلاج في المجتمع العربي بين رفض المريض والاستعانة بالمشعوذين بمختلف أشكالهم، وما يقوم به هؤلاء من ضرب وتعذيب للمريض لإخراج السحر أو الجن منه وقد يفارق الحياة بسبب هذا الأذى، وثم يأتي التردد من قبل الأهل في إيصاله للطبيب النفسي ويرجح كفة عدم العلاج ممانعة المريض، والواقع أن العلاج المبكر هام وضروري لإيقاف الحالة ومنع التدهور والتأثير على ظروف الحياة، وقد يتطلب الأمر إدخال المريض للمستشفى النفسي وأحياناً بصورة قسرية، وإذا كان متعاون وهادئ يمكن أن تتم المعالجة دون الحاجة لدخول المستشفى، وبعد التحسن لابد من التأكيد أن العلاج يستمر لفترات طويلة وقد يكون مدى الحياة، والعلاج النفسي الفردي والعائلي والجماعي يأتي لاحقاً للعلاج الدوائي وحسب الحاجة.

– وقد شهد العقدين الماضيين تطوراً هائلاً بالعلاجات النفسية وخصوصاً المضادة للذهان، مما أدى إلى تحسن النتائج في هذا المرض، ويبقى التحدي أن يتم الإلتزام والمتابعة، وعلى أساسه تتوفر حقن عضلية طويلة المفعول لضمان استمرار العلاج وعدم إيقافه والإمتناع عنه كما يحدث بالعادة.

– والسؤال المتكرر من الناس عن كيف يتصرفوا والمريض يرفض العلاج، هذا ما يجب عليهم بحثه مع الطبيب المعالج حتى لو رفض المريض الوصول للعيادات.
