شروط ترخيص صيدلية
1 – أن لا تقل المسافة بين المكان المراد ترخيصه و أقرب صيدلية عن 100 متر في أى اتجاه يعني مش لازم في نفس الشارع بل في أي اتجاه حتى لو كانت في شارع تاني.
2- الاشتراطات الصحية المطلوبة
ا- ان لا تقل المساحة عن 25 م2 بعد خصم البروزات ( الأعمدة و الفواصل).
ب- ان لا يقل الإرتفاع عن 2.7 م لكامل مساحة الصيدلية يعنى لو فيه صندرة مثلا لازم يكون الارتفاع تحتها لا يقل عن 2.7 م
ج – أن يكون المكان مرتفعا عن مستوى الشارع أو الأرض المجاورة ( المحلات الملاصقة أو الرصيف)
د- أن لا تقل التهوية ( الأبواب والشبابيك) عن 1/6 المساحة يعنى لو المساحة 30 م2 يبقى التهوية المطلوبة 5 م2 يعنى لو فيه باب 2 فى 3 يفي بالغرض
هـ – توافر مصدر للمياة مع وجود حوض ستانلس او حوض صينى + رخامة بجواره مع عمل مرايا ( جزء اعلى الحوض والرخامة ) بالسيراميك بارتفاع 60 سم
و- بالاضافة طبعا للبلاط و الدهان و التشطيب

ا- مستندات ترسل بالبريد السريع ( لحجز المكان أو قيد الطلب)
طلب ترخيص يسحب من المديرية
إيصال سداد رسم النظر ( حوالة بريدية بخمسة جنيهات باسم إدارة الصيدلة مديرية الشئون الصحية ب( محافظة)
صورة من شهادة الميلاد.
صورة من بطاقة تحقيق الشخصية.
صحيفة حالة جنائية.
رسم اهندسي معتمد من مهندس نقابى من 3 صور موضح به مسقط للموقع العام و مسقط أفقى و يبين عليه مسار أعمال الصرف الصحي
ب- مستندات تستوفي لاحقا باليد
صورة من مزاولة المهنة
شهادة الإدارة المركزية عن الملكية + شهادة الإدارة المركزية عن العمل بالحكومة بغرض فتح صيدلية بمحافظة ……………….
شهادة قيد لفتح صيدلية من النقابة العامة بعد الحصول على موافقة النقابة الفرعية.
صورة من عقد الإيجار الموثق بمحضر تصديق و الأصل للإطلاع.
كما تزود المؤسسات الصيدلية بآلاتي
3 دفتر صيدلية من الفجالة أو من النقابة لو متوفر

4 أختام كالتالى
ختم يحمل ” اسم الصيدلية وعنوانها “
ختم يحمل” اسم الصيدلية سموم“
ختم يحمل” اسم الصيدلية مخدرات“
ختم يحمل” اسم الصيدلية أدوية تالفة“
ثلاجة
طفاية حريق لا تقل عن 6 كجم
دستور الأدوية
الدمغات المطلوبة
عدد 6 تمغة فئة واحد جنية
عدد 9 تمغة فئة تسعون قرشا
عدد 11 تنمية موارد فئة عشرة قروش
عدد 3 مهن هندسية فئة واحد جنية
- دمغة مهن طبية من إدارة الصيدلة لكل صفحة من صفحات الدفاتر كالتالى
دفتر قيد التذاكر كل صفحة ب 25 قرشا
دفتر قيد السموم كل صفحة ب 50 قرشا
دفتر قيد المخدرات كل صفحة بواحد جنية
دوسية لحفظ الأوراق
